You cannot get a free copy of Microsoft Word, though you do have 2 options: You can download the Word Viewer for FREE or if you have a Gmail email address (which is free) you can download the file in Word format and send it to your Gmail account and then open it with Google Docs.

This first link is the form you would fill out on your first visit to our office. By downloading it now and printing it out you can fill it out at your leisure and save time at the office. We will have these forms at the office, so this is not required.
